THE APPLICATION DEADLINE IS DECEMBER 1ST.

• a pdf copy of your personal essay, which should include what you have done to demonstrate your interest in the health professions, particularly related to the area for which you are applying (job shadowing, etc.)
• a  pdf copy of a resume that includes:
  • work experience
  • honors received while in high school
  • extracurricular activities
• pdf copy of your high school transcripts
• pdf copy of your ACT/SAT Scores
• pdf copy of unofficial transcript(s) from dual-credit courses (if applicable)
• contact information for three people who can serve as a reference

It is the applicant’s responsibility to verify a completed application prior to the deadline.
